• Description:
• Tenders are invited for Institutions for All Kinds of Vehicle Insurance Applicants intending to register for more than one item should submit separate applications for each item and the application fee for each item is Rs. 500.00 and the application fee should be credited to Account No. 2026181 maintained at the Bank of Ceylon Kalutara Branch and the original receipt obtained should be submitted between 8.30 a.m. and 3.00 p.m. on working days and obtain separate application forms for each item and all applications perfected should be sent in a single envelope along with the certified copies of the Business Registration Certificate and the VAT Registration Certificate. All applications may either be sent by registered post addressed to “Office of the Regional Director of Health Services, District Secretariat Building, PO. Box 29, Kalutara” or hand delivered to receive on or before 23rd December 2022. The caption, “Registration of Suppliers for Year 2023” should be mentioned on the left-hand top corner of the envelope containing the bid.
