• Description:
• Tenders are invited for Purchase of Radiation Survey Meter 01 Bid Document Fee: Rs 3500/-. Bid Security: 75000/-. At the time of obtaining bid documents, bidder should submit the receipt obtained from the Accountant of the District General Hospital, Polonnaruwa or the receipt obtained upon crediting the amount to Account No. 7040277 maintained in favour of Director, District General Hospital, Polonnaruwa at the Kaduruwela Branch of Bank of Ceylon along with a letter of request on a company letterhead. Documents by interested and qualified bidder / representative or nominee. I. Written application on a business letterhead of the institution addressed to the Director, District General Hospital, Polonnaruwa II. Receipt obtained from the Accountant, District General Hospital, Polonnaruwa upon payment of the non- refundable bid document fee in cash as stated above. Bidders may obtain further information from the Accountant, District General Hospital, Polonnaruwa on Tel. No. 027-2225780 or 027-2222261 (Ext. 362) and the bid documents may be obtained from the Procurement and Cost Management Unit of the District General Hospital, Polonnaruwa only between 9.00 a.m. and 3.00 p.m. on working days until 15.11.2021. The name of the item should be clearly mentioned on the left hand top corner of the envelope containing the bid and the bids prepared in duplicate should be sealed and may either be sent by registered post addressed to "Chairman, Regional Procurement Committee, District General Hospital, Polonnaruwa” or deposited in Procurement Box No. 01 kept in the Office of the Accountant of the District General Hospital, Polonnaruwa to receive before 2.00 p.m. on 16.11.2021. Late bids will be rejected and the bids will be opened immediately after closing of bids (at 2.00 p.m. on the same day) at the Maithripala Sirisena Auditorium in the second floor of the District General Hospital, Polonnaruwa in the presence of the tenderers or their authorized representatives.
