• Description:
• Tenders are invited for Supply of Rubber Stamps. Date Stamps. Plastic and PVC Name Boards and Stickers Conditions and Terms: 01. A non-refundable registration fee of Rs. 1000.00 (Thousand) for one item and Rs. 200.00 (Two Hundred) each for each additional item and the receipt obtained from the Shroff of the Hospital upon payment of the registration fee in cash or by a Money Order drawn in favour of Director. District General Hospital. Embilipitiya" or a Cheque drawn in favour of “Director. District General Hospital. Embilipitiya* (subject to realization of cheque) should be attached and submitted with the application. 02. It is not necessary to call over at this office to obtain application forms. Application could be self prepared on A4 size paper and submitted. Documents pertaining to the payment should be attached to the application and the applications that are in conformity of the specimen will be rejected without any intimation. (Copy of the Business Registration Certificate should be attached to the application.) 03. Ouotations are invited as per the terms of the Procurement Guideline Code and at times of necessity, the hospital reserves the right to call for bids and effect purchases from suppliers other than registered suppliers. 04. Goods and Services supplied should be in conformity of the required standards and samples. 05. All applications should bo sent either by registered post or courier service or hand delivered and deposited in the Tender Box kept at the room of the Accountant to receive before 2.00 p.m. 05.12.2022.
