Road, Matale, between 9.00 a.m. And 3.00 p.m. On Working Days of the Week from 21.09.2021 until 04.10.2021. A Non-Refundable Fee of Rs. 2500.00 should be Paid in Cash to the Shroff of the Office of the Regional Director of Health Services, Matale and the Receipt Obtained should be Submitted to Obtain a Set of Relevant Bid Application. Bids Submitted should be Accompanied Either by a Bid Security to the Value of Rs. 25000.00 Obtained from a Commercial Bank Recognized by the Central Bank of Sri Lanka in Favour of the Regional Director of Health Services or the Said Amount should be Paid in Cash to the Shroff of the Office of the Regional Director of Health Services and the Copy of the Receipt Obtained should be Submitted.
