Duly Completed Applications Prepared According to the Specimen Appearing below should be Sent by Registered Post Addressed to Secretary, State Ministry of Provincial Councils and Local Government Affairs, No. 330, Union Place, Colombo 02 to Receive before 29.12.2021. The Words "Registration of Suppliers - 2022" should be Marked on the Top Left Hand Corner of the Envelope Containing the Application. Every Supplier should Pay a Non Refundable Fee of Rs. 500/= for each of the Items Seeking registration.this Amount could be Paid by a Money Order Drawn in Favour of The "Secretary, State Ministry of Provincial Councils and Local Government Affairs" (Receiving Office - Slave Island) or the Receipt Obtained for Payment of this Amount to the Shroff of this Office should be Submitted along with the Application.
